2025 HACEMOS National Scholarship Acceptance

Dear Applicant,

On Behalf of the 2025 HACEMOSScholarship Committee, we are delighted to announce that you have been selected as an Scholarship recipient.

In order to receive your scholarship award in a timely manner, we ask you to complete the following action items by Thursday, July 31st, 2025. 

1. Use this form to clearly state whether you accept or decline this scholarship. If you decline, no further action is needed.
2. Provide information about your enrollment for the 2025-2026 school year.
3. Provide the mailing address (family home, university dorm, college appartment, etc) where you can receive the HACEMOS Scholarship Foundation check. To ensure the check successfully arrives, we cannot send it directly to your university's financial aid office. Once it arrives, you must take the check to your financial aid office so it can be applied to your account.
4. Provide student headshot (by providing picture you authorize HACEMOS the use of material in social media platforms)
5. Provide a short testimonial sharing how HACEMOS scholarship has contributed to your academic development.
